1. Please fill in your name, email address and phone number.
2. In the “Message” area, simple write, “Smiles For Life”, then type any details that you like relative to the best times for your whitening appointment. For example, you might write: “Smiles For Life. If possible, I prefer a whitening appointment in the afternoon, on a Wednesday, Thursday, or Friday.”
3. After you submit the form, our Smiles For Life office coordinator, Shelley, will contact you directly to make your appointment!
